Electronic Pre-Participation Evaluation (e-PPE)

e-PPE is our Athlete Health Information Registry/Repository solution that has been enhanced to address the US requirements for youth participation in sports activities.

Pre Participation Evaluations ("PPEs") are mandated for student athletes within the middle, high school and college demographic. This process requires a student to submit a personal and family medical history, secure consent from a parent or guardian, and obtain clearance from a medical professional. In turn the school AND sponsoring State organization must supervise, collect and maintain copies of the relevant information for administrative purposes.  Within the United States alone, there are 35 million student-athletes with approximately 14 million requiring mandatory PPEs on an annual basis at the high school, club and university level.

Our solution ("e-PPE") automates the entire process offering significant benefits to all participants. Those benefits include time and cost savings for all stakeholders, improvements to standard of care and HIPAA compliant privacy and security of data.

e-PPE is a comprehensive on-line medical questionnaire/registry that can identify symptoms of health problems that may pose a risk for an athlete participating in sport and exercise. Developed and tested by sports medicine experts and used by thousands of high-level athletes, the e-PPE is now available to individual athletes, athletic trainers, team physicians, primary care physicians, and sport administrators.

e-PPE contains approximately 1500 branch chain questions establishing the gold standard of care, yet on average a student will complete e-PPE in 20 minutes from wherever they have a connection to the Internet.

e-PPE is architected to ensure sub second response times to literally tens of thousands of simultaneous users and 99.9999 percent uptime. e-PPE is currently available in French, English and soon to be released in Spanish. 

e-PPE reference sites include Stanford University, the University of Western Ontario, the University of Calgary, York University, University of Southern Maine, Minnesota State High School League and the COC CAMP.  In March of 2009, e-PPE was successfully implemented with FIFA in for the Under 17 Qualifying  Event in Algiers.


The e-PPE is a very powerful tool for obtaining important medical information that affects the health and safety of an athlete participating in competitive sport.  It includes the content recommended by the American College of Cardiology and the Preparticipation Physical Evaluation, Third Edition published by the;

Confidentiality of personal health information (PHI) is assured with the PrivIT system of privacy and the e-PPE is compliant with international privacy rules including HIPAA and FERPA (Family Education Rights and Privacy Act). The athlete has complete control over the sharing of their PHI data. After completing the questionnaire, summary forms can be printed to take to a physician or athletic trainer.

  1. Form A (Consent to Participate in Athletics) provides medical authorization for participation.
  2. Form B (Emergency Information) provides emergency medical information that can be kept on file by athletic trainers and sport administrators and used during travel. 
  3. Form C (e-PPE Medical History Summary) summarizes the positive and pertinent negative findings of the questionnaire. 
  4. Form D is a blank form for recording physical examination findings by a physician.
